Refer an Injured Worker

This page is for attending providers, claim managers, vocational rehabilitation professionals, attorneys, self-insured employers and their representatives, and other authorized referral sources. It explains what services we offer, what to include in a referral, and how the process works.

Injured workers can also start on their own by completing the “I am a patient” form at the same address.

Please do not send protected patient information by ordinary email. Use the secure referral form linked on this page. Email is for general questions only.
